Bereavement

Horizon's Bereavement program provides support to our hospice families and to anyone who has suffered the death of a loved one in the community. These services are available at both our Chicago and South Suburban offices. Grief support services are part of Horizon's Circle of Care. They are provided at no charge to our hospice families or to anyone in the community who may have experienced the death of a loved one. Though these services are free, donations are gratefully accepted.
Horizon Grief counselors work with you and your family to assist you in coping with your grief in any of the following ways:
- Thirteen-month follow-up with hospice families
- Eight-week support groups, including:
- Adult Loss
- Spouse Loss
- Second Year & Beyond
- BraveHeart Youth & Family Loss Group
- Individual counseling for children and adults
- School outreach, including:
- In-school counseling sessions
- Crisis support
We also provide Special Events & Workshops for your community group or place of worship, as well as professional and community education on topics such as:
- General Grief
- Grief in the Workplace
- Children's Grief
- Grief Literature
BraveHeart: Support for Grieving Children and Teens
Young people who have experienced the death of a loved one often need support to overcome their sense of pain and isolation. BraveHeart Youth & Family Loss Group provides a warm, supportive community of other children, teens and parents who are also coping with loss through death.
BraveHeart offers eight-week, age-graded support groups for children and teens age 5 through high school. In these groups, youth engage in age appropriate activities and discussion designed to help them build a supportive community and to cope with the loss in their lives. At the same time the youth are meeting, their parents, grandparents or other caregivers meet to gain support from one another and learn more about their child's grief process. BraveHeart holds one eight-week session in the Fall and another in late Winter/Early Spring in a South Suburban location. The sessions are held one weeknight per week, and begin with a light supper for all.
