Community Outreach
For your community organization or faith-based organization.
Building and sustaining relationships with faith-based and community organizations is the key to the success of the Community Outreach Program of Horizon Hospice & Palliative Care. The program provides education regarding end-of-life issues. We facilitate your education through interactive presentations, workshops and health fairs. Presentations, as well as any materials distributed at events, are free of charge. While we will happily customize a presentation for your organization, you will find several examples of presentations we have given listed below:
