CLINICAL TEAM MANAGER FOR INPATIENT HOSPICE UNIT
GENERAL SUMMARY: Responsible to for management of inpatient care services consistent with Horizon Hospice policies and procedures. Also responsible for the working relationship with Rush Medical Center.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Coordinates interdisciplinary care services.
- Ensures delivery of patient services and compliance with agency policy.
- Adheres to policies, procedures and guidelines to fulfill program objectives.
- Assures compliance with general IDPH, Rush Medical Center, Medicare COP’s and JCAHO regulations.
- Oversees scheduling, supervision and ongoing education of inpatient staffing.
Plans and implements specific clinical projects, coordinating them with overall agency plans.
Maintains agency compliance with state health department/federal government standards and regulations for licensure, home health agency and hospice certification. - Maintains the maintenance of medical records, assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards.
- Participates in orientation program for new hospice staff personnel.
- Participates in plans for recruitment and career development of patient care team personnel.
Assists with staff education program for all team members. - Provides guidance in communication, coordination, and effective operations relating to patient care through development and utilization of recording and reporting systems, manuals and other resource materials.
- Assists with utilization review activities.
- Assists in the development of personnel policies and monitors compliance in areas of responsibility.
- Provides input to related administrative/computerized systems and monitors the processing of data related to patient care.
- Participates in professional and community health and welfare activities.
Is a member of the Management Team. - Participates in other projects and/or provides information for a project outlined by the Director of Patient and Family Services.
- When needed take call on weekends.
- Assists with patient care when needed and appropriate.
- Participates in relationship building activities with Rush Medical Center.
- Serve on Rush committee as appropriate.
- Ensure 24 hour/7 day staffing and supervisory support for the inpatient unit.
- Responds to hospital referrals as needed and is responsible for access to inpatient hospice care at Rush Medical Center.
- Monitors and is responsible for inpatient budgets.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
- 3 years of experience in a hospice or other healthcare setting preferably Bachelor’s level education with a current professional RN license.
- Minimum one year of supervisory experience in an inpatient unit environment, Home health or hospice experience also preferred.
- Demonstrated ability including knowledge of health care policies and program responsibilities.

HOSPICE HOME CARE NURSE
GENERAL SUMMARY: Responsible for the care of hospice patients in accordance with the care plan established by the interdisciplinary team. Plans, coordinates, and manages services provided in the patient’s place of residence. Monitors the effects of care on patient/family health status, evaluates care provided and the need for revision. Acts as a liaison among the hospice patient, the attending physician, hospice medical director, volunteers and all members of the hospice interdisciplinary team.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Supervises the plan, schedule and execution of home care visits to hospice patients/families. Supervises regular visits made by hospice aides and licensed practical nurses.
- Provides documentation of care through charting and updates care plans as necessary. Responsible for closing records of discharged cases.
- Assures the coordination and facilitation of patient evaluation conferences as needed.
Concentrates on the area of pain and symptom management and is responsible for maintaining appropriate clinical and diagnostic skills. - Coordinates hospice home nursing care with other agencies participating in patient care.
Facilitates the admission and discharge of hospice patients at affiliated hospitals and in-patient facilities. - Assures effective integration between home care and in-patient services. Rotates participation with other hospice interdisciplinary team members in attending patient staffing at affiliated institutions.
- Participates in volunteers training, initial bereavement assessment, in-services for affiliated staff and other educational programs at the request of the management.
- Participates in the scheduled rotation for on-call coverage.
- Maintains a cooperative working relationship with the hospice physicians and clinical director, and keeps them informed about patient care; apprises the clinical director of personnel issues specific to the Aides and licensed practical nurse.
- Complies with Horizon Hospice policies, standards of practice and operational guidelines.
Serves as a hospice representative to those outside the program by sharing information about the program and the concept of hospice. - Demonstrates an understanding of state and federal regulations governing hospice care.
Participates in Horizon Hospice educational programs. - Participates in Quality Improvement activities as assigned.
- Performs other duties assigned by the Clinical Managers.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
- BSN (preferred) or RN experience in hospice care and care of the terminally ill.
- Current Illinois license.
- Community nursing and oncology background.

DEVELOPMENT ASSOCIATE
Serves as the primary staff for the creation of the organization’s informational products including printed materials, DVDs, videos, website and social media content.Provides staff support to the annual benefit and other events, and helps develop and staff the auxiliary group. Prepares press releases and related materials and works to secure media placement.

INPATIENT CERTIFIED NURSE' AIDE
GENERAL SUMMARY: An individual who, under the direction of a Registered Nurse, provides personal care service and homemaker to patients on the inpatient hospice unit within the guidelines of the interdisciplinary team care plan.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
1.Under the direction and ongoing supervision of the registered nurse, provides essential personal care assistance to the patient:
1a. With bath, care of mouth, skin and hair.
1b. In bathroom or in using bathpan or commode.
1c. In and out of bed as well as with ambulation activities as ordered under the care plan.
1d. With prescribed exercises which the patient and home health aide have been taught by appropriated professional personnel.
1e. Delivering meal trays and assist the patient with eating
1f. By taking a patient’s temperature, pulse, blood pressure and respirations if directed by a Registered nurse.
1g. With provision of wound care as directed by RN’s
2.Provides emotional support and compassionate care to the patient and family unit.
2a. Demonstrates listening skills.
2b. Offers encouragement when appropriate.
2c. Practices patient in voice and manner.
3.Maintains required competency and skills
3a. Documents initial and bi-weekly supervision and evaluation by RN on each case assignment.
3b. Participates in annual performance review.
4.Reports any changes in the patient’s condition or family situation immediately by notifying the RN
5.Practices Universal Precautions according to OSHA regulations and Horizon policy.
6.Adheres to all Horizon Hospice policies and procedural guidelines.
7.Attends a minimum of twelve (12) hours of continuing education programs or approved audiovisuals per calendar year.
8.Maintains current knowledge and practice of infectious disease protocols.
Contacts - Patients, families, volunteers, staff members, hospital/facility personnel, the public and household pets.
Physical Demands - Physical exertion-walking, lifting, pulling, driving, step climbing, flex hours and travel.
Environmental Conditions - Exposed to infections, communicable diseases, odors, blood, excreta and hazardous materials.
Working Conditions - Periods of stress-variable workloads and deadlines. Exposed to terminally ill patients and their families.
9.Participates in Quality Improvement activities as assigned.
10.Performs other duties as assigned by the Clinical Managers.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
A Home Health Aide must be certified by the Department of Health. The Aide must be current in all health requirements and meet federal home Health Aide training and competency requirements. This competency shall be demonstrated by passing a written competency and skills examination by a qualified Registered Nurse. All home health aides must submit to a criminal background check on application for employment.

INPATIENT HOSPICE LICENSED PRACTICAL NURSE
GENERAL SUMMARY: Responsible for the care of hospice patients in accordance with the care plan established by the interdisciplinary team under the direction of a registered nurse. . Observes and collects clinical data on the effects of care on patient/family health status in collaboration with team members.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Provides direct patient/family care as defined in the Illinois Nurse Practice Act.
Provides timely and accurate documentation of care. - Concentrates on the area of pain and symptom management. Observes and collects data on patient’s response to treatment/medications.
- Provides inpatient hospice care and services appropriately reporting identified needs to a registered nurse or other team members.
- Participates in the scheduled rotation for weekend and holiday coverage.
- Complies with Horizon Hospice policies, standards of practice and operational guidelines.
- Serves as a hospice representative to those outside the program by sharing information about the program and the concept of hospice.
- Demonstrates an understanding of state and federal regulations governing hospice care, in particular the requirements for general inpatient care.
- Participates in Horizon Hospice educational programs.
- Participates in Quality Improvement activities as assigned.
- Performs other duties assigned by the Clinical Managers.

- Graduate of an accredited practical nurse or vocation nurse program.

HOSPICE ADMISSION NURSE
GENERAL SUMMARY: Hospice Admissions Nurse is responsible for assessing hospice patients in the community and developing an initial Plan of Care for hospice appropriate patients.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Receives Intake information and provides assessment visit to establish a Hospice Plan of Care.
- Collaborates with case managers to provide clinical assessment and establishes hospice reimbursement according to published charges.
- Completes initial database, consents and initiates a Hospice Plan of Care.
Communicates clinical findings and Plan of Care to appropriate Clinical Supervisor and hospice team member. - Communicates the status of prospective patients with Admission Team.
Acts as resource to physicians, staff and community agencies relative to pain management, palliative care and hospice services. - Provides/coordinates hospice nursing care for patients receiving in-patient level of care in assigned hospitals.
- Participates in interdisciplinary team meetings and other meetings as required by the Admissions Coordinator.
- Develops and maintains working relationships with community agencies in order to promote and enhance Hospice services and continuity of care.
- Participates in education programs and Quality Improvement activities.
- Performs other duties assigned by the Admissions Coordinator.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
- Current Illinois license as a registered nurse.
- A minimum of (3) years nursing experience with hospice.
- Possess strong clinical assessment skills.
- Possesses good communication skills, both oral and written.
