Circle of Care Services

To help people in the last phase of their lives, Horizon Hospice offers an array of medical and supportive services:

  • Pain and symptom management provided by a hospice physician and hospice home care nurse in cooperation with the patient's primary physician.
  • Instruction and guidance for family and friends providing care at home.
  • Assistance with personal care, feeding and light housekeeping.
  • Spanish-speaking staff are available
  • Hospice-related medical supplies, equipment and medications.
  • Professional social work services and spiritual counseling.
  • Complementary and alternative therapies to improve physical, emotional and spiritual well-being, including acupuncture, aromatherapy, art and music therapy, healing touch, herbal remedies,  massage therapy, reiki, and pet therapy. For a complete description of these treatments, click here.
  • Bereavement counseling from staff and volunteers.
  • Help with funeral arrangements.

In providing these services, Horizon Hospice staff considers each patient's family, friends, caring neighbors and religious community as part of the "Circle of Care."

During an initial contact, Horizon staff ask questions to understand basic information about a patient’s state of health and social circle.  In a subsequent assessment visit, staff members gather more complete information. They then develop a care plan based on the stated wishes and perceived needs of the patient and family.

For more than a quarter century, Horizon Hospice has provided care to patients regardless of their ability to pay.  Most hospice services are covered by private insurance, Medicare or Medicaid.
