Horizon Hospice and Palliative Care

Chicago's first hospice, Horizon's mission is to provide comfort for the dying and to preserve dignity at the end of life.

Horizon delivers comprehensive care to terminally ill patients and their families throughout the Chicago area, primarily in their homes or residential facilities. Our Circle of Care services range from pain and symptom management to household assistance to grief counseling--all delivered by skilled, compassionate healthcare professionals and well trained volunteers. Established in 1978, Horizon brings more than a quarter-century of experience and expertise in end-of-life care to people facing a life-limiting illness.

As a not-for-profit, community-based organization, Horizon is directed by a local volunteer board of directors. We're committed to providing high-quality patient-centered care to people in need--regardless of age, race, religion, diagnosis, or economic status. Spanish-speaking staff are available.

Memory Cloths
Participants will craft their own memory cloth--a collection of patterns, images, and objects symbolic of their loved one. All have the option to share their work with the group.
