Farr Curlin, MD, Assistant Professor of Medicine, University of Chicago, is Team Physician for the South Suburban office of Horizon Hospice and Palliative Care. Dr. Curlin received his MD degree from the University of North Carolina, and did his internship and residency at the University of Chicago Hospitals. He is board certified in internal medicine, and he has a special interest in medical ethics.

Andrea Bial, MD, received her MD degree from the University of Illinois College of Medicine. She did her residency at the University Hospital Case Medical Center –Case Western Reserve University School of Medicine and her fellowship at The University of Chicago Hospitals. She is board certified in internal medicine and geriatric medicine.

Jacqueline Cameron, MD, received her MD degree and also did her residency at Northwestern University Medical School. She is board certified in internal medicine and hospice and palliative medicine.

Joseph Shega, MD, received his MD degree from Northwestern University. He did his internship and residency at the University of Pittsburgh and his fellowship at the University of Chicago Hospitals. He is board certified in internal medicine and geriatric medicine.

Donna Myers, MSN, BSN, CNP, is the Palliative Care Program Coordinator. She received her MSN from Rush University and is board certified as a gerontological nurse practitioner. Ms. Myers received her BSN from DePaul University.

Carol Harms, MSN, BSN, CNP, received her MSN from Northern Illinois University and is board certified as a family nurse practitioner. Ms. Harms is also certified in Hospice and Palliative Care. She received her BSN from the University of Kansas Medical Center.

Horizon Hospice contracts with the following Children's Memorial Hospital Pediatricians:

David M. Steinhorn, MD, received his MD degree from the University of Minnesota, and his post graduate work was done at the Medical College of Virginia and the University of Minnesota. He is board certified in pediatrics, pediatric critical care, and hospice and palliative medicine.

Elaine Morgan, MD, received her MD degree from the University of Pennsylvania School of Medicine. She did her post graduate training at the Children's Hospital of Los Angeles, Children's Hospital Medical Center/Dana Farber Cancer Center, Boston, and Children's Memorial Hospital in Chicago. She is board certified in pediatrics, pediatric hematology, hospice and palliative medicine and medical ethics.

Joel Frader, MD, received his MD degree from Tufts University School of Medicine. He did his post graduate training at the Children's Hospital of Philadelphia, and at the Tufts New England Medical Center. Dr. Frader is board certified in pediatrics.
