Ways To Give

Horizon loves its donors. As a not-for-profit, 501(c)3 organization, Horizon Hospice and Palliative Care relies on the generosity of the community to provide the highest quality palliative and hospice care to everyone in need of services.

Your donations make it possible for our work to continue. One tenth of our annual budget comes from individual, foundation and corporate donations. Horizon also hosts fundraising activities throughout the year, such as the annual Circle of Care Benefit and the annual South Suburban Chef's Extravaganza.

Your generous support provides pediatric palliative care, bereavement counseling for grieving family members, and hospice and palliative care for those who may fall through the cracks of the Medicaid/Medicare reimbursement system.

We deeply appreciate your financial partnership through individual donations of cash or stock. These donations can be made in memory of a loved one, or in honor of someone special in your life.

Should you prefer that donations be made in memory of your loved one in lieu of flowers, Horizon can provide envelopes for the funeral or memorial service. If this is your wish, please have the funeral director contact us

Challenge Grant

Horizon is the grateful recipient of a challenge grant from an anonymous funder to match donations through June 15, 2013 from new donors as well as increased contributions from previous donors.  If you have not contributed to Horizon in the last two years, please consider a donation.  The challenge grant will maximize the impact of your contribution. If you have contributed within the last two years, please consider increasing your donation. Matching funds will be awarded for contribution up to $6,250 per person/year for a total of $50,000 over two years.

Gifts of Stock or Bequests

Should you elect to make a contribution of stock, please consult your financial agent before doing so to ensure that capital gains taxes do not apply.

If you would like to make a contribution to the future of Horizon through a bequest, we would be honored to discuss this option with you and your financial representative.
